Changing Your Name
Two pieces of documentation are necessary for proof of a name change. These pieces of documentation include a social security card as well as either a valid driver’s license or valid passport. Both of these pieces of documentation must have the new name reflected within them.
To update your information, please mail or fax your change of name to the address or fax number given below. The Graduate School Change of Name form is also available for printing and faxing or mailing. When you submit your request, please be sure you include:
- Your former name and your new name
- Your current address and phone number
- Departments you have taken classes in
- Current student status
- YOUR SIGNATURE
